How do I contact the practice?

How do I join the practice as a patient?

Please complete the new patient application form and send it to admin@ichc.com.au for our team to process.

How is an appointment booked?

Visit our ‘Schedule Appointment’ page and then book via the widget on the page or via the HotDoc app.

For new residents to the practice, they will need to book a ‘New patient’ appointment. All existing patients can book a ‘Standard’ or ‘Long’ appointment depending on the number of issues which need to be addressed.

Why are fees being introduced for Integrated Clinical Health?

To sustain a viable practice, delivering excellent care able to recruit and retain excellent staff.

What about pension card holders?

Pension card holders will also incur a gap payment. Additional services including allied health may incur additional fees.

When will the consultation or visit be billed?

On completion of the consultation, the Medicare rebate will automatically go back to the nominated account.

Do you offer telehealth/phone appointments?

Yes, these can be booked online via the ‘Schedule Appointment’ page. These appointments are particularly useful for simple tasks, follow up reviews, medication changes, etc where a face-to-face appointment is not necessarily needed.

What about care plans, medication reviews and health checks?

All these services are bulk billed.

What if I or my next of kin are not able to book the appointment online?

In this case, the consult will be completed by the GP and the next of kin/resident will receive a text message via the HotDoc application and an email for payment after the consultation.

What about an urgent situation?

This will be dealt with immediately.

What if a patient is at the end of the life and in need of care?

All these GP services are bull billed.

When would a multi-disciplinary team (MDT) meeting occur?

If there is a change in a resident’s condition which is out of character, a multidisciplinary team meeting will be organised to look at early intervention and prevention to correct any issues. Any further action is agreed between patient, family, and clinical staff.

How is the MDT meeting organised?

This will be coordinated by Integrated Clinical Healthcare. The meeting can be in person or via telehealth services.

Can I keep my own Geriatrician?

Yes, resident-centred decision-making is at the heart of our Practice, and you can use any clinician. Our team will coordinate your care across all services.

What if I want to book in with the on-site skin clinic?

Dr Steven Ma will visit the home on a nominated day. All residents will be invited for a skin review. The consultation fee is $160 which includes all on-the-day procedure costs.
